TEACH-BACK DOCUMENTATION must reflect what the client DEMONSTRATED, not just that teaching occurred or that the client 'verbalized understanding.' WHY THE DETAIL MATTERS: Generic documentation ('education provided', 'verbalized understanding') provides no evidence that the client actually learned and can perform the skill safely; detailed documentation: (1) creates a legal record that competency was verified; (2) communicates to other providers what was taught and to what level; (3) identifies what follow-up education may be needed; (4) demonstrates the nurse's use of evidence-based teaching practice. DOCUMENTATION ELEMENTS: Teaching method used (teach-back, demonstration); what was demonstrated; level of competency demonstrated; client's questions or concerns; any knowledge gaps identified; plan for follow-up education. This is a JCAHO and CMS expectation for patient education documentation.
